Director of Information Technology (IT)
Seeking a Director of Information Technology. The position will be based in the Stevens Point area. This is a newly created position due to company growth and expansion. The Director of IT will be responsible for managing and streamlining the operation of the IT department and ensuring alignment with the business objectives and long-term strategic plan of Family Health La Clinica. Will provide direction and support for IT solutions that enhance mission-critical business operations, and work closely with decision makers in other departments to identify, recommend, develop, implement, and support cost-effective technology solutions for all aspects of the organization.
Qualifications: Bachelor’s Degree in Information Systems or another related field required. Minimum of 5 years of management and leadership responsibility specifically within IT required. Healthcare experience strongly preferred. Experience in analysis, implementation, and evaluation of IT systems and their specifications. Experience with budget and reporting on expenditures.
Dental Assistant - Stevens Point
The Family Health La Clinica Dental Office of Stevens Point is seeking Dental Assistants for full time hours. Become a member of our skilled and compassionate Dental team! The role of a Dental Assistant is to provide chair side assisting to the Dentist by performing the following duties: . Prepares patients for dental care services, sterilizes and disinfects instruments, sets up instrument trays, prepares materials and assists dentist during dental procedures. Ensures patient needs are met in a comfortable, professional & timely manner. Prepares, tracks, and sterilizes equipment in a timely manner. Assists in recording medical & dental histories. Exposes and develop diagnostic dental x-rays. Preparing quotes of DSS Performs routine equipment maintenance and dental operatory cleaning and preparation. Assures adequate inventory of necessary supplies and instruments are in their proper places in the operatory rooms and other areas of the clinic to support that day's work. Pours, trims, and polishes stone models. Assists dentist in management of medical and dental emergencies. Instructs patients in oral hygiene and plaque control programs and provides postoperative instructions to patient as prescribed by Dentist. Records treatment information in patient records Maintain aseptic techniques. Applies topical fluoride to child's teeth as prescribed by the dentist, including plaque control techniques. Assist in communicating necessary treatment and fees and in arranging financial agreements with patients. Day shift with variable hour Mon-Fri.
Qualifications: High School diploma or equivalent required. Previous Dental Assistant experience or completion of a Dental Assisting program required. Must be able to work in a fast paced setting. Strong communication skills and computer skills required.
Site Specialist - EPIC
This position is responsible to be the application expert, provide end user training and support, technical support, for testing and for maintaining FHLC-managed components of the Epic software.
Provide direct support to clinical operations and patient care teams to facilitate the integration and optimal use of the Epic application in the clinic. Collaborate with clinic leadership and users to identify opportunities for efficiencies in clinic operations, clinical practice support, and improved health outcomes. Provide daily support to Epic applications users through troubleshooting, coaching, and consulting. Identify, assess, and troubleshoot application and equipment issues as they occur, owning the problem and seeing it through to full resolution. Refer issues that require additional assistance to the member’s IT/Help Desk or OCHIN’s Help Desk, including technical and workflow information necessary to resolve the issue. Communicate resolution to the user when completed. Maintain member-managed system setup such as the provider master file, security, and preference lists, SmartPhrases for service area. Manage error queues and In Basket pools established for member organization. For EMR, work collaboratively with clinicians and in concert with established EMR processes to manage requests for additions of diagnoses and procedures codes to the system. Manage the requestor’s expectations until request is satisfied. Requests for additional system-level charting tools from the review process through followup communication to users. Coach the user through creating their own SmartPhrases. Collaborate with OCHIN and member oversight or advisory workgroups to optimize use of the system through implementation of new Epic PM and EMR features and upgrades. Collaborate with OCHIN to test new features and upgrades prior to implementation. Attend appropriate clinic department meetings as Epic expert and as OCHIN liaison.
Training: Demonstrate outstanding interpersonal communications skills and adult teaching techniques. Coordinate and provide all necessary training to new clinic staff and providers, coordinate issuance of ID and password, and provide go-live support according to established processes. Provide competency/efficiency training for existing users. Support upgrade training in collaboration with OCHIN and Member organization. Coordinate with clinic leadership and physician champions for brown bag lunch sessions (or established staff or provider meetings) as forums for interest-raising, problem-solving, or training topics, and provide expertise and any handouts to support the event. Collaborate and coordinate with OCHIN and any Member support teams, including Help Desk, to translate learnings and experiences into documentation, training aides, newsletters or other materials appropriate for your clinic.
Other Expectations: Attend monthly OCHIN Site Specialist meetings with peers for best practices, information sharing to and from OCHIN PM/EMR/IT resources, continuous training, and sharpening of support skills. Attend other workgroups as appropriate – Billing, Reporting, Operations Council. Attend annual OCHIN Learning Forum.
Qualifications: Bachelor or Associate’s degree preferred. Will consider equivalent relevant work experience in lieu of a formal degree. Previous EPIC (EMR) experience preferred.
Population Health Coordinator, RN - Wautoma
Family Health La Clinica, a growing Federally Qualified Health Center, based in Wautoma WI, is seeking a Registered Nurse for the position of Population Health Coordinator. The Population Health Coordinator will coordinate team-based care to provide health services and education to individuals and families through effective team-based care and partnerships with patients, their caregivers/families, community resources, and their providers. Facilitates a “shared goal model” to achieve coordinated high quality care that is patient and family centered. Other duties will including leading the Wisconsin Health Plus care including phone calls, scheduling., roster review and updates; health promotion, collaboration on patient care and education specific to both chronic and acute care; outreach; assist with department project management and QA/QI initiatives; and serve as the point of contact, advocate and resource to patients.
Qualifications: Registered Nurse in the State of Wisconsin with a graduate of nursing school, BSN preferred. Minimum of two years of experience interacting with patients providing counseling, nursing care and education. Computer/typing skills required. EMR experience. Knowledge or experience in some or all of the following areas: Asthma Education, Cardiovascular Health, Diabetes Education, Nutrition, Smoking Cessation, AODA Intervention/Counseling/Referral. Demonstrated ability to function both as a team member and as an independent service provider in a health care setting.

Registered Nurse, MSAW - Wautoma
This position will focus on providing nursing care, prevention education, and other services as a means of reducing the risk and slowing the transmission of COVID-19 within Wisconsin’s Migrant Seasonal Agricultural Worker (MSAW) population and their employers. Assist with the deployment of initiatives to bring education, resources and guidance to partners, agricultural farms, and migrant camps to support the MSAW’s during the COVID-19 pandemic. Will assist with COVID-19 surveillance and screenings.
Provides guidance to those that are leading the development of and support of health screenings, and the leveraging of additional clinical resources for expanded screenings related to COVID-19. Assistance to the team to link and refer MSAW’s and their employers with appropriate health care and social service/community resource networks. Collaborates with clinicians in developing and delivering health information, nursing care and education specific to both acute problems (COVID-19 related) and chronic problems of patients presenting at the mobile health center (i.e. COVID-19 symptoms and co-morbidities of Diabetes, Hypertension, Back Pain, Diabetic Foot Care, Nutrition, etc.) Learns and understand the mobile health center workflows with respect to optimal and coordinated health care for Migrant, Seasonal, Agricultural population. Conducts timely outreach tailored to meet each patient’s condition specific needs. Serve as a point of contact, advocate and informational resource for patient, family, care teams, and community resources for the MSAW population. Maintain records & appropriate documentation in various computer programs, OCHIN EPIC. Adhere to OSHA & HIPAA requirements. Provide Spanish/ English interpretation (if bilingual) for monolingual providers and patients. Assist with notifying patients of results and recommended action. Ability to work collaboratively in a team-based approach with clinical and non-clinical staff, patients and patient’s families. The position is expected to spend a minimum of 50% time in the field, working with Wisconsin growers and Migrant and Seasonal Agricultural Workers throughout the state of Wisconsin.

IT Help Desk Technician - Wautoma
The Help Desk Technician’s role is to ensure proper computer operation so that end users can accomplish business tasks. This includes receiving, prioritizing, documenting, and actively resolving end user help requests. Problem resolution may involve the use of diagnostic and help request tracking tools, as well as require that the individual give in-person, hands-on help at the desktop level.
Duties will include the triage of incoming help requests from end users via both telephone and e-mail in a courteous manner. Determine priority, create, and track tickets in FHLC IT system. Ensure tickets are updated with detailed technical notes. Prioritize and schedule service on IT problems. Escalate problems (when required) to other appropriate IT staff. Provide technical assistance for all IT related hardware (phones, AV equipment, printers, PCs, copiers, webcams).Set up and manage various user accounts, including new hires, terminations, and new group email accounts. Troubleshoot various types of hardware issues and provide maintenance support for workstations, printers, scanners, switches, and various peripherals. Setup, image, configure, and deploy desktops/laptops and other IT equipment when needed. Provide remote and phone support for staff in satellite offices and various locations using remote access tools. Assist with new user training and orientation. Assist with asset management. Assist with creating and updating knowledge base articles. Work to reduce the number of escalations by performing and receiving cross-training and knowledge transfer. Assist with shipping, receiving, and inventory ordering, as necessary. Identify and help implement opportunities for improvement. Participate in the call rotation, if necessary. Assist with user / office moves, as necessary. Perform other duties as directed. Day shift Monday - Friday.
Qualifications: Minimum HS Diploma or equivalent. Associate degree in computer science, information systems, or equivalent preferred. May consider equivalent and related work experience in lieu of a formal degree. One-year previous experience in HP, Windows environment, troubleshooting and performing software maintenance; an understanding of the business process; excellent problem solving and interpersonal skills; and a hands-on, interactive approach to delivering information systems support desired. Experience providing IT support within the health care industry a plus.
Dental Hygiene Assistant - Wautoma and Beaver Dam
The primary responsibility for a Dental Hygiene Assistant is to assist the Dental Hygienists and other dental staff in the delivery of patient care in a professional manner. Ensures that hygiene appointments are timely, thorough, safe, and a positive experience for every patient.
Prepares rooms for dental hygiene services, sets up instrument trays, and prepares materials as needed to assist the hygienist during dental hygiene procedures. Seats and dismisses patients when needed. Provide excellent customer service to help ensure a smooth and comfortable visit for the patient. Assists in reviewing and recording medical history with patient/parent, up-dating medical and prescription information, entering this data into the computer and passing it on to providers. Performs routine dental operatory cleaning and preparation following OSHA infection control guidelines to maintain a safe environment for all. Sets up the room for the next patient according to the schedule. Assures adequate inventory of necessary supplies and instruments are in their proper places in the hygiene rooms and other areas of the clinic to support that day's work. Reviews Hygienist schedule and help maintain patient flow throughout the day. Be aware of scheduling changes and prepare accordingly. Communicate with front desk staff as needed. Review the schedule for the next day. Communicate any concerns with the hygienists so they can be addressed prior to the appointment. Assists with regular maintenance of the dental equipment and prepares work orders if needed. Maintain a professional atmosphere. Adhere to confidentiality, state, federal, and HIPPA laws and guidelines with regards to patient’s records. Other duties as assigned. Day shift Monday - Friday.
Qualifications: High School diploma or equivalent is required. Previous dental assisting or dental experience helpful, but willing to train the right person. English required. Bilingual in Spanish or Hmong is preferred, but the not required.

Qualifications: Master's degree in a behavioral health field and current Wisconsin licensure as a LCSW, LPC, LMFT, Psychologist or other related licensure required. Program management experience required. The Case Manager hired must complete training in mind-body medicine through the Center for Mind Body Medicine. Previous experience as a Case Manager is preferred. Education and experience in mind body medicine modalities preferred. Bilingual in English and Spanish is preferred but not required.

A Dental Hygienist with Family Health La Clinica implements dental hygiene and prevention program which maximizes patient contact and treatment completion and moves patients into routine recall status. Performs dental measures designed to preserve teeth and prevent the spread of oral disease. Duties may include:
Perform oral prophylaxis as prescribed by dentist including cleaning of calcareous deposits, accretions and stains from teeth and beneath margins of gums. Perform periodontal treatment and maintenance as prescribed by dentist. Examines gums for signs of disease and does routine oral cancer screening of mouth, chin, neck and lymph nodes to detect swelling, tenderness or other symptoms or abnormalities. Charts conditions of decay and disease for diagnosis and treatment by dentist. Applies fluorides and other cavity preventing agents as prescribed by the dentist. Update and maintain current health histories noting areas of medical concern. Exposes and develops diagnostic x-ray film. Provides clinical services and health education to improve and maintain patient oral health. Conducts dental health clinics for community groups to augment services of dentist.
Qualifications: Graduate of accredited dental hygiene program. Current RDH in the State of Wisconsin. Excellent communication skills.
